Popularmente conocido como “culebrilla”, el herpes zóster es una enfermedad causada por el virus varicela zóster, el mismo que provoca la varicela.
El primer contacto con el virus se suele tener en la infancia y se manifiesta como una varicela. Tras esto, el virus permanece latente en el cuerpo y puede no causar problemas durante muchos años, pero determinadas situaciones pueden provocar su reaparición en la piel dando lugar al denominado herpes zóster.
¿Cuáles son los primeros síntomas de la culebrilla?
1. Los primeros signos de herpes zóster son ardor o dolor punzante, hormigueo y picazón, generalmente en el tronco, el muslo o en la cara (zona más delicada porque puede afectar a la vista o al oído). El dolor puede ser leve o severo.
2. Al cabo de 4 ó 5 días la zona presenta enrojecimiento y comienzan a aparecer erupciones o vesículas de forma agrupada. Durante esta fase las lesiones son altamente contagiosas pues el virus se encuentra dentro de las vesículas.
3. Alrededor de una semana después las lesiones comienzan a secarse, y la enfermedad acaba por remitir.
Toda persona que ha pasado la varicela es susceptible de padecer un herpes zóster; 1 de cada 4 lo sufrirá a lo largo de su vida.
Neuralgia post-herpética
Aunque puede aparecer a cualquier edad, su incidencia aumenta significativamente con los años, especialmente a partir de los 50, cuando el sistema inmunitario comienza a debilitarse.
Además de prevenir el herpes zóster, la vacuna reduce de forma significativa la probabilidad de desarrollar complicaciones. La más relevante es la neuralgia posherpética, un dolor persistente que puede durar meses o incluso años y afectar de forma importante a la calidad de vida.
También puede ayudar a evitar otras complicaciones menos frecuentes, como infecciones oculares o problemas neurológicos.
La vacuna del herpes zóster: quién debe ponérsela
- Personas a partir de 50 años: la edad es el principal factor de riesgo. A partir de esta etapa, el sistema inmunitario pierde eficacia y aumenta la probabilidad de reactivación del virus.

Las vacunas frente al herpes zóster disponibles actualmente han demostrado un buen perfil de seguridad. Los efectos secundarios más habituales son leves y transitorios, como dolor en la zona de inyección, enrojecimiento o febrícula.
Tratamientos para el herpes zóster: ¿cómo aliviar el dolor?
Por lo general la enfermedad se resuelve espontáneamente en una o dos semanas. En pacientes inmunodeprimidos o personas mayores puede ser necesario un tratamiento con fármacos antivirales que se debe iniciar de forma temprana para que sea efectivo.
Desde octubre de 2014 está disponible en España la vacuna para la prevención del herpes zóster y la neuralgia post-herpética para mayores de 50 años con prescripción médica. Se recomienda prioritariamente a pacientes crónicos con EPOC, diabetes o insuficiencia cardiaca.
